By logging in to MemberConnect, you can quickly and easily find a broad range of helpful information and resources. You can use MemberConnect to:

Check Benefits and Coverage

On the left side of the page, under Check Benefits and Coverage, you can get details about your plan, claims and authorizations, including:

• coverage levels
• out-of-pocket costs
• claims status
• charged and covered amounts for each service item
• status of your authorizations, by service date and provider
• any past correspondence you've received, such as privacy policy information, explanation of benefits, or previous authorization letters

Find a Provider

Use our convenient online provider directory to locate a provider in your area.
To search our vast network of more than 55,000 providers, type or select your search criteria and you'll see a list of doctors, hospitals, groups and clinics near you.

Explore Health Information

Because we know our members have questions about their health, we offer Achieve Solutions®, an award-winning behavioral health and wellness Web site.
Here you can find a wealth of in-depth information including:

• educational articles, news and tips to help you make positive health decisions
• self-assessment tools to help you identify and address health concerns
• community resources to compare hospitals, locate child and family care, and much more

The Forms Area

The Forms area gives you access to the forms needed in order to file a claim, authorize use or disclosure of medical information, or register as an outpatient.
The forms are in PDF format and are printer friendly. In addition to frequently-used forms, this section also includes sample forms, instructions and tips designed to help you complete them accurately for best results.
